What are Community Support Services (CSS)?

Services to support older adults, seniors and adults living with disabilities to remain safe, independent and healthy at home. 

What is AccessCSS?

AccessCSS is a network of trusted non-profit organizations funded by Ontario Health offering Community Support Services across Ontario Health East.

Receive Confirmation

If you request a service online you will receive an email with details about the appointment or service you have requested.

Your Appointment

The provider will contact you and work with you to get the support you need.

Services are available throughout Eastern Ontario. E-Referral is available in Champlain and growing across the South East and Central East regions of Ontario

Injury

We believe that access to community home support services should be quick and easy. 

  • Who can access Community Support Services through AccessCSS?

    Anyone living in the following regions: 

    • South East Ontario 
    • Champlain 
    • Central East Ontario (coming soon)
  • Can I apply for a family member or loved one?

    Yes! You can request service for someone else, but they must be aware of the request and agree to let you send it.

  • Do I need a physician or healthcare provider to refer me?

    No, you can refer yourself for AccessCSS services or ask your doctor for help.

  • What happens when I apply to the AccessCSS Navigation Service? What can I expect?

    You will be asked to complete a few questions  so we can get a general sense of what you're looking for. The form takes about 10 minutes to complete. 


    After submitting your request, one of our staff members will contact you by phone or email. Specify your preferred method of communication. 


    If you need a service our partners can’t provide, we will do our best to link you to another local service.
